Who Is Most at Risk?

Mechanism: Vitamin A toxicity (hypervitaminosis A) is dose-dependent and cumulative. Topical retinoids absorb systemically at rates between 10-30% (varies by formulation, vehicle, skin condition). Adding oral vitamin A or precursors dramatically increases total body retinoid exposure, triggering toxicity symptoms: hepatotoxicity, teratogenicity (in reproductive-age females), bone loss, CNS effects (headache, pseudotumor cerebri), and severe dermatitis.

Recommendation: ABSOLUTE CONTRAINDICATION for isotretinoin + any oral retinol/vitamin A supplement above RDA (700 mcg for women, 900 mcg for men). For topical retinoid patients, counsel to avoid oral retinol supplements entirely. If patient requires oral vitamin A for medical reason (e.g., night blindness, specific deficiency), pause topical retinoid.

Salicylic Acid + Benzoyl Peroxide (High Concentrations)

Severity: MODERATE RISK

Medications/Ingredients Involved: Salicylic acid (2-3%) + benzoyl peroxide (5-10%)

Mechanism: Both are keratolytic and antimicrobial. Concurrent use, especially at higher concentrations, can strip skin barrier excessively, deplete natural oils, and trigger irritant dermatitis. The combination is often promoted in acne products but warrants caution in individuals with sensitive skin or barrier dysfunction.

Medications/Ingredients Involved: Hydroquinone (2-4% OTC, up to 4% prescription) + vitamin C serum (L-ascorbic acid 10-20%)

Mechanism: Hydroquinone is a tyrosinase inhibitor that addresses melanin production. Vitamin C is an antioxidant with mild tyrosinase inhibition. Combined, they theoretically enhance depigmentation efficacy. However, both agents can irritate skin, and the low pH required for vitamin C stability may enhance hydroquinone irritation.
